I have used the At-A-Glance pocket calendars for a few years now. That works great for me because it fits in the back pocket of my pants. It also has a monthly view at the back where I write appointments. I use the weekly view pages to write notes. I also carry a small pen with me. It has been a life saver several times.

Before the pocket calendar I carried a pocket size composition book, which is a smaller version of the composition books used in science classes. It worked well but I stopped using those after I starting using the pocket calendar.

The spiral bound notes always ended up with the spirals getting bent or broken but they are easier to write in and tear out pages as needed compared to the composition book.

The index card idea is great also. I learned that trick from a co-worker (that probably reads Zen Habits). I use the index cards for work. I write down on an index card a task that has been assigned to me. I keep two index card boxes--one for blank cards and one for completed cards.

When a task is completed I date the card, write the results of an issue resolved and I file the card. I work in Information Technology but the pen and paper method has worked great. I have a BlackBerry and a Laptop but for a capture tool nothing beats the pen and paper method.
